Bad casting, or did something let loose and jam it?

This motor was put together some months ago and put in a frame so I run the cam in in at 2200 rpm for 20 mins and then I could run it for a few minutes once a week or so to keep it turning over or whenever I had the need too hear the errantic lumpy idle of an angry grey .....just to keep me enthused . Anyway all went to plan, run the motor in , started and run it for a few minutes every week ....I have always had the air filters on it and its been in my shed since rebuild .
Last week I cranked it over after a few seconds it barked into life run for a minute or so and heard a thump then it immediately started blowing and fuming out the breather and walking stick ...I shut it down immediately ............sat down and had a think about it .......yep something real wrong i thought .........so off comes the head .......just as thought 1 x broken piston ( no 5 ) , 1 x piston ( no 6 ) with a nut imbedded , and 1 x piston ( no 4 ) marked and pitted .....there was some small fragments of steel in no 5 ...nothing recognosiable ............the head had a 3 thou impression of the nut on the flat surface ....nothing much else visible at this stage .
The nut was stainless steel which I used for the carb mounting to the manifold ...no nuts missing on the manifold ......no nuts or hardware missing anywhere .....where the f**k have these come from .........could they have been in the manifold since build, bearing in mind its been run for 20 mins at 2200rpm and had its neck wrang every week ever since .........or something sinister maybe .......

...full inspection coming soon..........Fingers
Don’t you find it Funny that after Monday(M) and Tuesday(T), the rest of the week says WTF?
